Default Strange modem 629 error

I've got a laptop running Win 98 SE. 192 MB RAM. It has been scanned
for viruses and spyware. There have been no programs added to my
computer in at least a month.
For the last 3 weeks, as soon as I try to connect to an ISP, I get an
Error 629 "Disconnected by remote computer." The program doesn't even
attempt to get a dialtone. This happens with different phone numbers,
and with both Dial Up Networking and HyperTerminal. However, Phone
Dialer is able to dial out and establish a call with the numbers, so I
know the modem works.
I have reinstalled the modem drivers as well as DUN. The problem
persists. I have been through all of the troubleshooting ideas at
modemsite.com.

Looking at the modem's log, I see a difference when the Phone Dialer
connects and when DUN tries to connect:
Phone Dialer:
Send: ATX3DT2115557655cr [I changed the # for this post]
Recv: ATX3DT2115557655cr
Recv: crlfOKlfcr

HyperTerm (or DUN)
Send: ATDT##########cr [This is exactly what is displayed]
Recv: ATDT2115557655cr
Recv: crlfDELAYEDlfcr

Any ideas?

Thanks!

Is it REALLY easier to write a long post and copy text from
windows and wait for who knows how long for an answer which may
or may not come and may or may not be correct than it is to
Google for
"Error 629" "Disconnected by remote computer"
???

Some telecom authorities prohibit excessive redialing of the same
phone number. If a modem detects this, it may temporarily "blacklist"
that particular number. Depending on your modem's chipset or firmware,
you may be able to disable this behaviour.

See http://www.modemsite.com/56k/blacklist.asp

As for your "failure to connect" problem, this is not a consequence of
the "DELAYED" message, rather it is the cause of it, ie too many
retries.
