Default MSConfig StartUp - Cleanup Button

Can someone explain to me how to use the msconfig, startup, cleanup button
and what to do to cleanup un-used or not working lines in the start up
section ?

I have lines such as; istsvc, optimizer, web rebates, etc...... Hiw do I get
these out of the startup side of msconfig ? DO I un check the lines that I
use all the time, and check the ones I do not have, or want in the start up
side of msconfig ? Using the clean-up button, what does it really do, and
will I mess up my system if I click on it ?
