Default Duplicate Icons in Control Panel

My system has been halting alot as of late. About six monthes ago I noticed I
had two TELEPHONY icons in my control panel. Both are active ( I can use
either one and they mirror each other in content) since i hadn't noticed any
performance issues i just let it be. In January a second SYSTEM icon showed
up (same scene mirror, use either...) Two weeks ago a second ADD HARDWARE
icon showed and as stated I am halting often, almost daily or more to be
true. I am up to date on my AV updates (V-com) and am behind a firewall. With
the new icon i can no longer use my usb ports. plug anything in and total
freeze. Plug in prior to start up and i get "windows protection error" then
reboots to safe mode only if i unplug the device. This isn't a bios issue
since my other bootable HD works w/o any issues when i unhide it (bootmagic)
any way i'm hopelessly lost and confused and a bit frustrated so any help
will be welcomed
system particulars a
Athlon 1150
750 RAM
WIN 98se
Also I halt on any number of apps after any number of minutes or hours
uptime no pattern that i can see. The most common set of details closely
follows this...

UNKNOWN caused an invalid page fault in
module unknown at 0000:bff80ecb.

the module will differ but the recommendation to restart my system after
closing all apps remains a constant and usually this requires a three finger
salute, the reset button or the power button.
sorry bout the length just want an informed public
any takers? please

Non-specific errors in different applications at random times suggests a RAM
problem, or a similar hardware issue. Check that the fans are working and
that nothing is overheating, check that the RAM modules are properly seated,
and run an extensive system diagnostic test.

Thanx for the reply but my system stays very cool. Both fans (tower) run
fine and are cleaned weekly the cpu fan is a supercooler and the memory is a
ok. also i dont have these issues when running off the second HD as my boot
drive (accepts USB, no dup icons , no halting or freezing and that is what
has me stumped. any other thoughts PLEASE feel free to ask or suggest and I
truly thank you for your time and thoughts thus far
p.s. i do have MANY specific error/application instances saved i just used
the unknown one from lack of knowing which of the others to choose let me
know if these would help

That sounds promising. What was the result from the system diagnostic test?

i use SystemSuite5 and it says all systems are fine really the only thing
that it cant tell is the usb ports. since i cant plug anything in it cant
really test em so.....
but even with the system info it comes up empty as well everything ok but
then it halts.
lol.
siezed for about 4 minutes then blue screen came up but said terminate
program or reboot but as i turned to get a pen to write down the error it
when on without my pressing ANY key it returned to windows and no problems i
cant even tell you what was shut down obviously something in the backround .
