Default Need help connecting router

Greetings! I would appreciate some really basic help in connecting a new Netgear Model WGR612 v7 router.

Where do I find out the following info?

Thanks in advance.

Enter your Internet IP Address
1. Get Dynamically From ISP (this choice doesn't work)
or
2. Use Static IP Address (where to find this info?)
-- IP Address
-- IP Subnet Mask
-- Gateway IP Address

QUESTION 4
Domain Name Server (DNS)

1. Address Get Automatically From ISP (this choice doesn't work)
or
2. Use These DNS Servers: (where to find this info?)
-- Primary DNS
-- Secondary DNS

QUESTION 5
Router MAC Address

1. Use Default Address (this choice doesn't work)
or
2. Use Computer MAC Address (this choice doesn't work)
or
3. Use This MAC Address (where to find this info?)

normally Dynamically (DHCP) is the right choice,
often a static address costs more (but can be usefull if
you are running a server that you want to have an ip-adress
that don't change over time), and as steve say
if you have a static ip you should have got some info from
your internet provider... on some "welcome"-paper perhaps?

The reason that it doesn't work for you is probably something
else, perhaps your internet provider belongs to the stone age
and requires you to use PPPoE or something like that? you should
have got some login/password info from them if so...

but you can set it manually if you like, to the values
you got from the ISP (or from a third-party DNS-provider).
it is the ip-number to the server that translates
names (like "thepiratebay.org") to numbers ("83.140.65.11")
so it is kind of important that it allways work
the secondary is a reserve if the primary doesn't work.

router mac addess normally doesn't mather.
but perhaps you must restart the router after you change it.

Thanks, folks. I did find the info at the ISP website, but it took some effort.
Here's the easy way...

For Windows 2000
start run:
ispconfig

For Windows 98SE:
start run:
winipcfg
