Default System recovery

I was forced to use the system recovery disk, but now when
I'm trying to enter my Certificate of Authenticity #, the
computer is requesting 5 blocks of information, however,
my Certificate of Authenticity only has 4. Please help.

Typically the likes of Packard Bell PC's that use a restore CD don't
require a product key to be entered during the restore process, PB
code it into the CD.
So you need to contact the manufacturer for assistance, [you don't say
what brand it is].
