Fix: Use a head cleaning disk.
2. Loose cable between drive and motherboard.

6. 3 1/2 inch 1.44 meg drive is defined as 5 1/4 inch
1.2meg drive ( or some other wrong type)
Fix: reset disk type in BIOS
