TWO copies of explorer
 
When I press ctl+alt+del, the task manager shows TWO copies of 'explorer' in
the list.

Many times when I (finally) succeed in closing the extra copy, it comes
right back (list shows two copies 'running'/)

What causes this and how do I permanently cure this?

If you have more than one Explorer window open, you'll have "Explorer"
in the task list for each window.

Click Start Find File or Folders, search the C: drive for files named
explorer.exe, and post back with their sizes and locations.

Also:
Update your anti-virus app and then run a full-system virus scan.

Use CWShredder, the CoolWeb removal tool, available he
http://aumha.org/downloads/cwshredder.exe
Close all browser windows and open apps, start CWShredder and click the Fix button.

In addition, install Ad-Aware SE free Personal Edition, start it, click its 'Check
for Updates' link in the app to install updates, then use it to scan your system,
and remove what it finds.

select Software Environment - Running Tasks

What do you see?

Note that some malware can cleverly disguise the name, eg the "ell"
could be replaced by a "one".

I'd suggest searching for exp?orer.*

Some malware attempts to masquerade as Explorer.exe by replacing the
"ell" with an "I" or a "one".

It could also be that the app name you see in the Ctrl-Alt-Del window
is not related to the app's filename.

There is no 'running tasks' choice shown.

When I ctl+alt+del I see 32 'things' running (in Norton Crashguard) - do not
recognize most of the names as there is nothing in the name to indicate (to
me) which program it refers to.

(None of the names look like explorer (iexplore shows when I open windows -
sometimes.))

TWO copies of explorer
 
As Frank mentioned the differences, you can check them he
http://castlecops.com/StartupList.html

Type the .exe's name in the search text input box and click search, start
with iexplore. The query will pull everything with that name if it is in
their database along with the description. Might as well check those .exe
files seen in Norton CG.
